Read this consent carefully to understand how Tablet uses telehealth technology to deliver medical and mental health services. By continuing, you confirm that the statements below are true and that you wish to receive care through Tablet’s virtual platform.

Informed Consent Regarding Use of Telehealth

BY CLICKING "I AGREE," CHECKING A RELATED BOX, USING ANY OTHER ACCEPTANCE PROTOCOL PRESENTED THROUGH THE SERVICE, OR OTHERWISE AFFIRMATIVELY ACCEPTING THIS CONSENT, YOU ACKNOWLEDGE THAT YOU HAVE READ, ACCEPTED, AND AGREED TO BE BOUND BY THIS CONSENT. IF YOU DO NOT AGREE, DO NOT CREATE AN ACCOUNT OR USE THE SERVICE. YOU HEREBY AUTHORIZE ANY PERSON WHO INDICATES ACCEPTANCE ON YOUR BEHALF TO ACT WITH AGENCY AUTHORITY.

Purpose

The purpose of this Telehealth Consent ("Consent") is to describe how telehealth works and obtain your permission to receive medical and/or mental health services from physicians, physician assistants, nurse practitioners, therapists, and other licensed professionals (collectively, "Providers") through Tablet's online platform (the "Service"). The term "you" refers to the person using the Service. When the Service is used on behalf of a minor between thirteen (13) and eighteen (18) years of age (or the higher age of majority in your state), "you" also includes both the parent or legal guardian providing consent and the minor receiving care.

Use of Telehealth

Telehealth uses secure communication technologies to connect you with Providers who are not in the same physical location. Tablet's platform may be used for diagnosis, treatment, follow-up, and patient education, and can involve:

  • Secure electronic transmission of medical records and notes
  • Photo images, personal health information, or other data shared between you and your Provider
  • Real-time audio and video visits
  • Asynchronous secure messaging and email communications
  • Review of medical device data, sound files, and video files
  • Remote monitoring and assessment of health conditions

In-person care may be available to you at any time. Discuss all available options with your Provider to choose the approach that is best for your needs.

Anticipated Benefits

Telehealth through Tablet may offer the following advantages:

  • Convenient access to licensed clinicians on your schedule without the need to travel
  • Reduced exposure to other patients and potential illnesses by receiving care at home
  • Ongoing, secure communication with your care team for timely follow-up
  • Greater consistency with treatment plans that can improve outcomes over time
  • For mental health services, support with managing stress, anxious thoughts, communication skills, and emotional regulation

Potential Risks

Telehealth relies on technology and remote communication. The following risks may occur:

  • Limitations of virtual examinations that may require an in-person referral for certain conditions
  • Technology issues, including interruptions, delays, or failures that may affect care or data transmission
  • Security events that, despite safeguards, could impact the privacy of electronically stored or transmitted information
  • Regulatory limits in some jurisdictions that may restrict certain prescriptions or services
  • Reliance on the accuracy and completeness of information you provide remotely
  • For mental health services, temporary emotional discomfort that can accompany therapy conducted virtually

Emergency Situations & Follow-Up Care

Telehealth is not appropriate for emergencies. If you require immediate or urgent care, call 911 or visit the nearest emergency facility. Tablet Providers monitor secure messages but may not reply immediately. For non-emergency concerns, contact your Provider through the Service or schedule an in-person visit when recommended.

Data Privacy & Protection

Tablet uses secure network and software protocols to help protect your information. Your health data may be shared with third parties only as permitted by law—for example, for treatment, payment, operations, or mandatory reporting obligations. Email and other electronic communications may involve some risk. Tablet cannot guarantee the security of services that you use to access your email or other communications.

Laboratory Products & Services

Some Tablet Services include at-home diagnostic testing provided by independent laboratories. All laboratory testing carries limits, including the potential for false negatives, false positives, or inconclusive results. Errors in sample collection or testing could affect your Provider's ability to diagnose or treat you accurately. Your Provider will review results with you and recommend additional testing or in-person evaluation if needed.
